Tennessee operates its own state OSHA program (TOSHA—Tennessee Occupational Safety and Health Administration) under the Tennessee Department of Workforce Development. Tennessee Board Rule 0440-02-.12 establishes communicable disease prohibitions: "No patron with definite open sores, exhibiting symptoms of infectious or contagious disease or disorders of the skin, or parasitic infestations will be served in a shop or school unless written permission from a physician has been secured." The rule also states: "No cosmetologist, esthetician, shampoo professional, instructor or natural hair stylist who knowingly has an infectious or contagious disease or parasitic infestation in a communicable stage shall give service in a school or shop." The Board mandates that infection control rules be posted and enforced in all establishments. Board violations carry fines, probation, license suspension, or revocation.

TOSHA enforces Tennessee's bloodborne pathogen standard, which is substantially equivalent to Federal OSHA's 29 CFR 1910.1030. TOSHA requires annual training covering esthetician-specific exposures (waxing, extractions, dermaplaning, microneedling, chemical peels), Hepatitis B vaccination, written exposure control plans, and 30-year exposure records. TOSHA also offers bloodborne pathogen training seminars across Tennessee, ranging from half-day to four-day programs.
